Project objectives are: - To provide reliable information and support understanding of chronic/invisible health conditions - To reflect on expectations and obstacles from both sides (from the side of young people with chronic health conditions and those working with them) - To understand relevance of continuous communication and cooperation of everyone involved for the success of the certain project, same as to emphasise relevance of empathy of those who work with young people with fewer opportunities - To raise the awareness about relevance of risk management and crisis intervention in international setting while including young people with chronic health conditions - To provide quality theoretical and practical step-be-step guidance for development of risk management and crisis intervention plans for each phase of the project (planning, preparation, implementation, follow-up) - To provide space for networking and support in development of new projects within Erasmus+ frame, where gained competences can be directly implemented and which can increase inclusion of young people with fewer opportunities By having this we aim to support and (especially in the long term) increase participation and inclusion of participants with fewer opportunities, specifically young people with chronical health conditions. Above everything, this aims to be done by building the capacities of professionals who directly work with young people and increasing the capacities of organizations working with young people, especially at international level.
