Since 2015, the ERHR (Equipe Relais Handicaps Rares), is a resources team working to consolidate the journey of the people with multiple disabilities all along their life. Part of a larger health and social organization, called les PEP 69/ML, and financed by the health ministry, the ERHR team is an essential stakeholder of the national network that aims to accompany the inclusion process of people with multiple disabilities. The ERHR has specific knowledge in networking as close as possible to the people and at training professionals in the health and social sector. The ERHR wishes to continue developing skills about empowerment, an essential concept in youth social work, very new in the French Health and Social Sector. The Association JAG was created in 1992 and was involved in driving the implementation of the assistance reform in Sweden. The reform became a reality in 1994 and meant that people with extensive disabilities were entitled to personal assistance. Personal assistance was then one of the foundations of empowerment. In 1994, the User Cooperative JAG started to organize personal assistance for the members of the Association JAG who wanted it. Today, JAG Personal Assistance is Sweden's largest private assistance provider, run without profit. The National Association JAG still works towards the same goal - that the members of JAG should be able to live in community with their family and like others in society, with the best personally adapted assistance possible. The JAG believes that every single person has the will and ability to exercise autonomy, given that they are surrounded by people who have the ability to understand and respect their will. Every single person has the right to communicate through their own ways. It is in this context that the ERHR and the JAG Association did choose to collaborate around Swedempowerment, in order to raise knowledge and skills around empowerment and self-determination of youngsters with complex disabilities. The aims of the SWENDEMPOWERMENT project was : 1. Understand the context and the environment of swedish youngsters with disabilities 2. Analyse how the Swedish ecosystem is built around these youngsters 3. Identify and challenge the resources and tools that people with complex disabilities have at their disposal to promote their self-determination. 4. Develop local and international partnerships
